TeleHeartGuard is launching its implementation with the aim of strengthening cardiac care through modern telemedicine solutions and cross-border cooperation.
The TeleHeartGuard – Post Acute Coronary Syndrome Tele Heart Monitoring project is implemented within the framework of the Interreg VI-A IPA Greece–North Macedonia 2021–2027 Programme.
Through the cooperation of four hospitals in Greece and North Macedonia, the project focuses on the safe monitoring of selected patients after their discharge from hospital.
The use of wearable devices and digital tools aims to support continuity of care and strengthen the connection between patients and their care teams.
